Service Description: | If you meet the eligibility criteria for the Family Nurse Partnership Programme your midwife will forward your details onto the Family Nurse Partnership Team.
Family Nurses work with clients who are:
First-time mothers aged 19 years and under at conception Live in the East Riding. First-time mothers aged up to 24 years if you are a care leaver or have special educational needs or disability A specially trained Family Nurse will then get in touch with you to say hello and arrange an initial visit with you at home to discuss and explain what the programme involves and how it all works. You’ll have a chance to ask questions and decide if the FNP programme is for you.
If you decide you would like to join the programme, you and your Family Nurse will meet regularly, at times that suit you. Your Family Nurse will visit regularly from early in pregnancy and can continue until your child turns two years of age.
You will share different ideas at each visit with your Family Nurse and decide together what topics to talk about. Examples of some of the topics you will cover are: preparing for labour and delivery, parent-child attachment, feeding your baby, immunisation, child development and school readiness. The programme is personalised to suit your needs so that the visiting schedule and content of the programme is tailored to you.
We know that dads are important to their babies, so we welcome dads, partners, friends or other family members to join in during the home visit if you would like them to.
You will have a dedicated Family Nurse for the length of the programme so will have a familiar and consistent professional to work with you during your parenting journey.
The Family Nurse will also carry out the all the health checks and developmental reviews that are part of the normal Healthy Child Programme
