Service Description:
The Integrated Specialist Public Health Nursing (ISPHN) Service provides the Healthy Child Programme, Safeguarding and Public Health from the 28th week of pregnancy until your child leaves school and for those children with special needs the service continues until 25 years.

The service deliver mandated contacts for under 5's, as well as Early support and assessment and referral to the Early Support Panel for children with additional needs; this also involves ongoing signposting to other services and support as required by individual families, children and young people.

The ISPHN Service contributes to the development of Education Health Care Plans (EHCP) and establish partnership and collaborative working systems with families and other agencies to support families.

The ISPHN service is a universal service for all families, children and young people resident within the East Riding Local Authority area and those who attend schools within the East Riding area.
